How to Submit Your Request

What Happens After You Submit

If we have everything needed, Checkmate will block the requested information within 4 business days after receiving your complete submission. 

If we need additional information to process your request (limited to certain items, like confirming identity or identifying the specific adverse items), we will notify you within 5 business days. 

We will make a final determination regarding whether the submission is complete within 25 business days of receiving your submission.

FAQ Section

1) I’m a victim of human trafficking. How can I block items from my report that are the result of trafficking?

You can submit a request by mail to one of the addresses listed above or by email to compliance@checkmate.tech.

2) What proof of identity is required?

To help confirm your identity, please provide one or more of the following:

1. A copy of a government-issued photo ID (e.g., driver’s license, passport), and/or
2. A document that shows your name and current address (e.g., recent utility bill, bank statement)

Helpful details (if available):

1. Full legal name (including middle name/initial and suffix if applicable)
2. Any other names used previously
3. Current and recent address(es)
4. Date of birth and/or last 4 of SSN (if you choose to provide)

If Checkmate has previously prepared a report about you, it is helpful (but not required) to include:

• Order Number / Applicant ID, or
• The name of the employer/landlord that requested the report

3) What documentation is required for a trafficking-related request?

Your submission should include documentation supporting that you are a trafficking survivor and that the adverse information resulted from trafficking.

For the most up-to-date examples of acceptable documentation and a sample request letter, refer to CFPB guidance.

4) What are “adverse items of information”?

These are the specific negative items you want blocked from being included in reports. Examples may include items related to coerced debt, housing instability, or other negative records that resulted from trafficking.

Please be as specific as possible about what you want blocked (for example, name of creditor/landlord, court case number, record type, dates, etc.). This helps us identify the exact items quickly.

5) What if my request is missing information?

If we need additional information to move forward, we’ll contact you (using the contact details you provide) and explain exactly what’s missing and how to submit it.

6) How long will this take?

1. Within 4 business days: If your submission is complete, we will block the identified adverse items.
2. Within 5 business days: If we need additional information (limited reasons), we’ll notify you.
3. Within 25 business days: We will make a final determination on completeness.
